Every client will have a free consultation where all needed services will be discussed. Once the consultation is completed an Individualized Care Plan will be created.  The ICP will include a list of services to be provided along with the number of personal touches that will be completed in a month. Monthly services will be continually reviewed and adjusted to meet a client’s particular needs.  Progress reports can be sent to approved family members on a weekly or monthly basis.  Services that may be included in a client’s care plan are as followed:

  • Home visits

  • Medication management

  • Vital sign monitoring

  • Home safety/equipment checks

  • Meal planning

  • Scheduling doctors visits

  • Accompanying to doctor visits

  • Social outings/shopping

  • Down sizing

  • Long Term Care Policy paperwork and billing

  • Sort mail and assist with monthly bills

CCA can also garner research on a patient's illness or condition, physicians, best practices and newest treatment options. CCA will work as a liaison with healthcare providers, insurance policies, and or family members.
